Children's Rehabilitation Service

Children's Rehabilitation Service ( CRS ) is a statewide organization of skilled professionals providing quality medical, rehabilitative, coordination and educational support services for children with special health care needs and their families. Every county in Alabama is served through a network of 14 community-based offices.

This network provides specialty clinic and other services by physicians, nurses, social workers, physical therapists, speech-language pathologists, audiologists, occupational therapists, registered dietitians, secretaries, and administrators.

CRS also collaborates with local school systems and families to address the complex issues associated with disabilities and special health care needs and provides education, training, specialized services, and assistive technology and devices to enable children with disabilities and special health care needs to attend, benefit from, and remain in school. Through services such as counseling, specialized therapy, and assistive technology for children and families, as well as disability training for teachers and other school staff, CRS offers the support vital to classroom success.

NEEDS ASSESSMENT SURVEYS REQUESTED:

The Alabama Department of Rehabilitation Services and Children’s Rehabilitation Service are encouraging families to participate in needs assessment surveys.
This CRS Family Survey focuses on health and related services that may be needed by children and youth with special health care needs. It also addresses issues that show how well the system of care in Alabama works for these children and youth and their families.
Your answers will help in the planning of better services and supports.
Children and youth with special health care needs (CYSHCN) have or are at increased risk for chronic physical, developmental, behavioral, or emotional conditions, and require more health and related services than most children. For this survey, please respond if your CYSHCN is between the ages of birth to 26 years.
The CRS Youth Survey will focus on work, education, recreation, health care, and independence. Your answers will help in the planning of services and supports for youth with special health care needs in Alabama.
CRS implements Alabama’s children with special health care needs program, mandated by Title V of the Social Security Act. The surveys are a portion of the federally required five-year needs assessment and are available at www.crsfamilysurvey.com for families and www.crsyouthsurvey.com for youth.
The surveys should take about 20 minutes or less to complete, and your answers will be anonymous. You may skip any question that you prefer not to answer. Beginning this survey implies your consent for CRS to use your responses in the summarized findings. Any direct quotes used in the final report will be anonymous.
